Transform Patient Care Beyond the Bedside

What is Chronic Care Management by Nurses?

In a world where healthcare is evolving at a rapid pace, Chronic Care Management (CCM) stands out as a beacon of patient-centered care. Nurses, the unsung heroes of the healthcare realm, are at the forefront of this transformative movement. They are not just caregivers; they are advocates, coordinators, and educators, making a profound impact beyond the bedside.

With our quick-to-digest but impactful course, participants will embark on a journey to understand the intricate dynamics of CCM programs within the United States healthcare system. From unraveling the complexities of fee-for-service versus value-based care reimbursement models to identifying the ideal patient profile for CCM services, this course equips nurses with the knowledge and skills needed to excel in modern healthcare delivery.

What you'll avoid:
1. Fragmented Care: Say goodbye to disjointed healthcare experiences for patients with chronic conditions. Our course empowers nurses to provide holistic care coordination, ensuring seamless transitions between healthcare settings.

2. Missed Opportunities: No more missed chances to improve patient outcomes. Participants will learn to leverage CCM services effectively, maximizing the potential for preventive care and early intervention.

3. Financial Uncertainty: Bid farewell to financial instability in healthcare practices. By mastering the nuances of value-based care, nurses can contribute to the financial sustainability of healthcare organizations.

4. Lack of Engagement: Say no to passive healthcare delivery. Our course emphasizes active nurse engagement in CCM, fostering meaningful patient-provider relationships and enhancing patient satisfaction.

5. Ineffective Care Coordination: Leave behind ineffective care coordination practices that hinder optimal patient care. Participants will learn evidence-based strategies to streamline care processes, reducing redundancies and improving efficiency.

What you'll get:
1. In-depth Understanding: Gain a profound understanding of CCM programs, from their historical evolution to their impact on patient-centered outcomes and healthcare costs.

2. Practical Insights: Acquire practical insights into the implementation of CCM services, including qualifying criteria, patient engagement strategies, and interdisciplinary collaboration.

3. Enhanced Skills: Develop essential skills for delivering high-quality CCM services, such as care planning, medication management, and health coaching.

4. Professional Growth: Elevate your professional profile with specialized knowledge in chronic care management, positioning yourself as a valuable asset in today's healthcare landscape.

5. Positive Impact: Make a tangible difference in the lives of patients with chronic conditions, improving their quality of life and reducing healthcare disparities.

Who this course is for:
1. Registered Nurses: Registered nurses seeking to expand their expertise and advance their careers in chronic care management.

2. Nurse Practitioners: Nurse practitioners aiming to enhance their practice and provide comprehensive care to patients with chronic conditions.

3. Clinical Nurse Leaders: Clinical nurse leaders dedicated to improving healthcare delivery systems and optimizing patient outcomes through CCM.

Informative Course on CCM

The course was a great introduction to CCM. I really enjoyed the case studies and "pop quiz" during the lesson to keep you engaged in the material.
